A lot of people in such a state do die while straining on the toilet. No joke. Drug use can also cause constipation.

The most popular times for having a heart attack include during early-morning strenuous exercise or taking a tough crap on the toilet.



On farting to death - Not likely, but I suppose one could twist one's bowels if the pressure released was great/fast enough.

There is no suggest, he was a drug addict, and it killed him.

problem was how his autopsy was conducted and the various revisions over the decades. It's hard to gauge what really happened as his body was long release to the family and buried before any significant review was conducted into it the first time.

It's theoretically possible to build up internal pressure strong enough to blow your insides out your butt.

But that's a very high-level and complicated martial arts technique, and you'd have to actually want to kill yourself with a fart and do your full follow-through and everything. It's not really something that happens on accident, unless god reaches through the a/m barrier and makes it happen somehow.

Heart attack toilet strains on the other hand represent at least a few hundred deaths a year in most countries, often several thousand.
