It's more of an age issue

Head pats is not something one should attempt once a boy gets beyond puberty.

He would be on the right side of justified to viciously fight back; even repeated kicking the groin if the head patter was someone that the victimized boy could not win a fair fight against.

I dunno why some guys are averse to being head patted. My last crush was a guy that liked randomly giving me hugs in public.

I would say it's an insult and/or a very dangerous joke/gesture.
It's like trying to shake hands with someone who lost their hand.

The only exception I could imagine is for safety reasons such as protecting someone's head from hitting an obstacle or in a flirtatious way however they are far from being "head pats".

There is a BIG difference between a friendship hug and head patting.
