Ol' Painless (Predator) and Favorite Thing (T2) are great choices.

I love the Gatling Bike from Machete and the Crank Gun from Magnificent Seven as well. Another good crank gun scene is in Red Dead Redemption during the Fort Mercer slaughter.

I king of want to add the .50 cal belt gun from Rambo 4 and the Sentry Guns from Aliens, but they are both stationary maching guns. No rotating barrels.
