Most horrific game glitches?

What are the most horrific/creepy game glitches you've seen?

I remember playing Elder Scrolls: Oblivion and i entered a town to complete a quest, found the guy i was supposed to talk to and his eyes, teeth and lips were floating outside of his face.

In Oblivion when you talk to someone the game freezes and zooms in on their face so this guys disembodied eyes, teeth and lips flapping around has stuck in my head for years.

Also Fallout New Vegas i'm walking around and randomly there's an NPC stuck up to their waist in the ground and their body is glitching out so their arms are way longer than they are supposed to be and are flapping around uncontrollably like something out of a horror movie. I ran in the opposite direction as fast as i could.

Back in the day I played Pirates of Dark Water on the Sega Genesis. And to this day I still want to know if anyone else experienced this same glitch.

When I got to the Citadel level, 9 year old me was really spooked out by the entire haunted mansion setting. Later in the level, there were mummy-type enemies that would come out of coffins and spit acid at you. But the important thing was that when I attacked them from behind as they're coming out of the coffin, the mummy turns photo-negative.

It wasn't anything major, but it was enough to tip things over for me and it ruined the word "citadel" for a large portion of my childhood (I never played Half Life 2).

I vaguely remember some Vietnam game where my squaddies didn't die. Their jaws were blown off, their skin was shredded, their limbs were gone, but their captured AK-47's were floating before them.

I called them suffering soldiers. I don't remember the name of the game.

Fallout 3 and Fallout New Vegas, npc half stuck in ground. CSGO if there is not enough players in a match and bots plays with, found one or two maps have a glitch where bots will be stuck between two floors, like the fallout bug. one half top floor, other half other daggling bottom floor. not sure of its a glitch/bug or just a game design flaw, but some fps multiplayer games when there is one or two to many players then a two players spawn on the same spot, the one lives and the other dies, seen in a few games that happen.

New Vegas had the creepyist one with the head twisting Doc. But I have seen floating eye balls and mouth on several games from multiple devs. It was like watching Mars Attacks! again.
