When I got to the Citadel level, 9 year old me was really spooked out by the entire haunted mansion setting. Later in the level, there were mummy-type enemies that would come out of coffins and spit acid at you. But the important thing was that when I attacked them from behind as they're coming out of the coffin, the mummy turns photo-negative.
It wasn't anything major, but it was enough to tip things over for me and it ruined the word "citadel" for a large portion of my childhood (I never played Half Life 2).

The most horrific ones are getting stuck in infinite loading screens for me
