After launching ZDL you will need to go to the settings tab. Once in the settings tab you will see two sections Source Ports/Engines, and IWADS.
Lets forcus on the Source Ports first. Under the blank listings for source ports you will see the add button. Click on the add button to get the add source ports window, and then click the browse button. Now in the search window find where you have GZDoom.exe, select it, and then click the open button. The search window will close, and you can now set a name to the source port in the add source port window. After that just click ok, and now GZDoom is listed in ZDL.
Next click on the add button under the IWADs section. This is the same thing as before with the source ports only this time look for offical doom engine wads. In this case look for where you have Doom2.wad, and add it to the list. When you are done you can add other main doom engine wads to the list, or go back to the main tab.
Now on the main tab and under the External Files list click on the add button. You will now get a new search window that will allow you to look for any of the supported mod files you wish to play with. So in this case look for the brutal doom v20.pk3 file, and add it to the list. Note that you can add more then one mod to this list as long as they don't conflict with each other.
If you wish to play custom maps like the hell on earth starter pack look for the wad file, and just add it to the External Files list. This will allow you to play brutal doom with the starter pack maps.
Now you are almost done, and thus come the easy parts.
Under the main tab look for source port drop down list, and select GZDoom.
On the main tab under the IWADs list just click on Doom 2 so it will be selected as the wad you wish to play with.
Finally click on the launch button, and now you will see a window show up loading GZDoom with the mods together. You should now be able to play Brutal Doom. Just remember you will need to set any key binding you wish to use in the games options menu.

As for the other issues you may need to download an updated version of GZDoom. Just note that updating to GZDoom v3+ will require you to use Brutal Doom v20b R, or Brutal Doom V21 Beta.
