The "multiverse" basically says that different realities exist at the same time; it's a way to keep things less crowded and/or being able to have multiple versions of the same character.
I don't know about the comicbook and movie universes, but in the TV realm, "Arrow", "Flash", the Legends etc. are all from "Earth-1", although some characters have been taken from other Earths already. Supergirl is on Earth-38, so it's entirely separate.
It's already difficult to explain why the different teams aren't helping each other out a bit more (unless it's crossover-time, of course), but with Supergirl on Earth-38 we do at least know why none of the world-shaking events there are seen in Starling or Central City.
And while Earth-1 and Earth-38 are pretty similar to "our" real Earth, except for the superheroes added to it and all the stuff that comes with it, things have already deviated a bit on Earth-2, and we've already seen "Earth-X" as a Nazi-ruled world. So it's not just about characters, they can make completely different worlds.
Sometimes they also make fun of it, like having all sorts of different Harrison Wells.
