Hm. "The Forged Realms" would be my choice.

The forged realms sounds like some ancient creation of gods or powerful entities of old. It can be a setting where those beings are still there or long gone. And the state of the region and the structures in it may wary. Basically paints the picture of some epic landscapes with unnatural but astounding beauty regardless of whether it is in ruins or not.

The fading empire gives a more darker vibe. Like a corrupt, decaying and rotting landmass. We are talking undead, demons and degenerates, with all sort of vile scheming and suffering going on beneath a thin veil. Imagine a civil war or poverty and plague filled slums and stuff. Or you could go the other way and show a world slowly withering away with its glory fading in time or maybe the arabian nights or the prince/princess & knights theme.

The shattered nexus sounds like something for a more futuristic approach with magic and technology intertwined. A place of great power that has been destroyed or simply deactivated. Something that could open a gate or portal or maybe act as a weapon of mass destruction or perhaps a temple or something of great importance in lore.

Or something like that ;p. Atleast that is what popped in my mind when reading the names.
