Same here too. Even did separate years. That's what gave me a pic to it, but no link or description. Wonder if it was abandoned and never released or was just a demo that didn't pull through...
Here is an archive of PC Gamer magazines with related cover discs over at the Internet Archive website:

If not from PC Gamer, maybe theres a magazine in this more generalised collection that seems familiar with the cover disc:

Perhaps it was an old PC Format cover disc?
Only thing is, as far as I can recall it was single player only. I could be mistaken though, it was a long time since I played it.
Another option might be Hexen 2, which did have a multiplayer mode, axes and crosbbows, 4 choose able characters one of which was a melee type guy and one which was female. But I seem to recall it was first person not 3rd person.
I remember Hexen, beyong Heretic, made by 3D Realms. Was a fun game, but that's not it.
Dark Vengeance I'm not sure. It looks like it may be it.
Finally we found it, with a lot of help. XD
It isn't abandonware?
Seems like it. I just found it on some emulation site which have old pc games as well. Can't post a link here since Steam deletes it. I can't run this game on Windows 10, though. Trying to figure it out.
