Index USB Device Not Recognized is driving me insane!
I had this issue once before and I found this discussion:

And the following solution worked then:

"Hello,

I had the same problem: device descriptor request failed when plugging the Valve Index.

This solution worked for me : in Control Panel > Power Options > Advanced Settings > USB Settings > USB selective suspend setting : turned it to "Disabled"

However that has not worked this time, neither has power cycling, reinstalling Steam VR, Uninstalling and reinstalling drivers or switching different USB ports.

It's actually a complete joke that my VR works fine for ages, has one small issue that's fixed and then gets the same issue that I waste 2 hours of my time trying to fix. Considering how much this thing cost, I don't expect to have to learn to be a technician in order to be able to use it.

Does anyone have any other solutions before I just fork out even more money for a new cable and hope that actually works...?

Got to love a cable that just breaks for no reason eh! Well guess I'd best find some cash to get a replacement while I have an expensive paperweight lying around in the meantime :/
