If you want a better look (though even this isn't fool proof for reasons of its own), you can change the CPU graph in Windows Task Manager to "logical processors" instead of "overall utilization".
No, it just means the card's doing it's job. If the game starts stuttering just reduce some values in the graphics settings.

Wont really cause any lasting damage but it might overheat sooner and turn itself off to protect the CPU parts. so make sure to be cooling your PC when using it with such intensity. dont cover the PC fans because it will heat up faster

you want your GPU at 99% utilization when heavy gaming because that means your GPU is doing its job. nothing bad will happen, its a good thing.
By limiting FPS to a decent value (which is either within Free/G-sync range of your monitor (if applicable) or at 85-90% utilisation) then the other 10-15% will act as a buffer, if there are any hard to render frames, heavily reducing framedrops. And sitting below 95% utilisation reduces latency noticeably. It makes ~120-144FPS have as low latency as ~300 FPS (some situations may be different, but most follow this behaviour.)
It also keeps the GPU (slightly) cooler, which can mean either quieter fans, or higher boosting, which leads to more performance.
99% GPU usage isn't a bad thing, it's just not a good/optimal thing, in regards to gaming.
If you even remotely care about a nicer performance, you would limit FPS (with RTSS) and keep GPU utilisation at 85-90%.

Higher usage could produce some stuttering (small pauses) when playing, and/or higher temperatures.
But nothing that would damage or harm the computer, after all it was designed to be used.
Depends on his use case, sometimes higher FPS can help. :^)
In older games, he could be running into a CPU bottleneck, since they aren't coded to use that many cores (usually 1-4), so it would only show up as like 5-40% on a modern processor.
I'm not saying that's exactly the case, but it could potentially be it.
Just because CPU usage is low doesn't mean it's not bottlenecking, or limiting somehow.
AMD has a frame limiter in their driver too.
It will limit the FPS in the same way as RTSS.
Never use Vsync as a frame limiter.
Even with Vsync, you should use RTSS (or other applicable frame limiter) to get a proper smooth experience.
