Valve Index USB isn't being recognized

My Valve Index arrived yesterday, and I have not been able to get it to work properly due to my computer not recognizing the USB. When I plug the USB into the port, SteamVR recognizes the headset for a brief instant before disconnecting, and I get a message at the bottom right of my screen that reads: "USB device not recognized. The last USB device you connected to this computer malfunctioned, and Windows does not recognize it."

My attempts to fix this have been:
- I have restarted my computer
- plugged the USB into different ports
- unplugged everything and waited before replugging everything back in
- checked my graphics card as well as USB ops for updated drivers
- uninstalled and reinstalled both Steam and SteamVR
- plugged the USB into a different computer

The same error message appears every time. The problem, as far as I can tell, is that the USB is busted, but I'm posting here in the hopes that I'm wrong and that someone can help me through this.

Looking in the device manager, my computer refers to the USB as "Unknown USB Device (Device Descriptor Request Failed)". Information from the "Device not started" Events log within the properties is:

I don't normally have software issues, so I don't exactly know what all information I need to give for someone to recognize the problem, but I'll post whatever I need to.

EDIT: So just did a clean install of windows. Still no luck...
Edit 2: Got it working ish. Unplug all the wires in every spot, ie the mid connection. Play with the wires in your hands like your giving it a massage. Plug in everything and cross your fingers. Seems like many people are having this issue as well. Best of luck...

Just spitballing here:

Do you have all of your equipment plugged into one circuit in your home?

Try running several different locations to power the devices.

By no means am I an expert on this, I literally just saw a video wherein it's "necessary" to have a bazillion plugs - perhaps it's a power supply issue. Try it, if it works please say so, if not, well, at least it's something to know.

For the sake of trail and error, I tried plugging everything into a bunch of different sockets. No dice.

Did you do this for the wire leading into the HMD as well? Also, when you say that it was working-"ish", does that mean that the problems persisted afterwards? Because that pretty much confirms that the issue is a faulty wire (at least in your case), and that would definitely need to be replaced.

I went through the whole cable, I am not exactly sure where it started working. I said "ish", because I am not sure if or when the problem my occur again. I just used it for a few hours with no problems, though I do agree the wire is faulty.

I massaged my wires and failed to get so much as a blush out of it. Lemme know how long your system goes without an issue.

This solution worked for me : in Control Panel > Power Options > Advanced Settings > USB Settings > USB selective suspend setting : turned it to "Disabled"

Here's my experience with a Quest HMD. I tried a variety of USB plugs on the computer and got the same message. It turned out that the connection needed to be a "powered USB" hub - ie, one that would also charge the HMD as I was playing. The more passive data-only USB's (even a USB C connection) would not work. Until I found the powered plug with a SS symbol, it was no joy. Once I found that connection it worked like a charm.

I had this issue, fumbled around with connections for about an hour, and found this solution: disconnect all cables from pc (I just disconnected all the bits from the tri splitter, including power) -> Open SteamVR -> go to developer options -> under reset, click remove all USB devices (from what I understand this completely deletes all index drivers and makes windows forget the devices) -> restart pc -> reconnect headset.

Hopefully that'll work for anyone stumbling here in the future, and hopefully valve will fix whatever driver issue they're having.

Worked for me too! THANK YOU. Even though my problem was a bit more complicated, it worked. I do think keeping good care of your VR Cordes, in the long run, solves almost all problems.
