Need a new Graphics Card for 120hz?
I have an LG OLED42C3 incoming and using a GTX 1070 Ti.
I mainly just play BG3.

A. Do I need to upgrade my graphics card to achieve 120hz on this TV monitor?
B. Wil I notice any difference in BG3 between what I can get now (60hz?) and 120hz?

Boom, you'll probably get 50-60 FPS
It shouldn't look bad on that TV given how good its picture quality is in general, and the panel.
1080p usually looks fairly decent on 4K TVs since it has the same aspect ratio and it's 1/4 the size so it's not an odd-ball size that would end up providing you with odd-ball looking pixels.
I will try that later to help see if I feel like I really need a faster GPU or not - thanks for the suggestion.

120Hz TVs are considered gaming panels with much better input lag and support for VRR/G-Sync/FreeSync.

VRR makes a huge difference, but you need to reach at least 48FPS to get into its range on most TVs.

Headaches are normal but last only a few hours. People experienced the same when they were switching from 14-inch to 17-inch, then to 21-inch, and eventually to 27-inch and 32-inch screens. People used to say I was crazy when I was using a HUGE 27-inch LCD TV, while others were still on 15-inch monitors, and TVs usually came in 21-inch sizes.
I do agree I get fatigue after playing BG3 for a while, but thought it had more to do with my previous 49" screen, which was one of my main reasons for dropping to 42".

So I'm gonna pic up a new card for my existing PC (not going to upgrade the whole PC just yet I don't think).

Up here in Canada after tax (Cdn dollars), I can get a 4070 Ti for $1250 or a 4080 for $1800.

Which would you recommend I go for understanding that likely one day I would end up transporting it into an upgraded rig?

Disable HDR and it will work.

I only have 2560x1440 as the highest resolution under the PC section (regardless of being in HDR mode in Windows).

Are you using an HDMI 2.0 cable?

No matter what, you should be able to get at least 4K30Hz regardless of the settings, unless you are using a cheap HDMI cable from a Blu-ray player (1080p30Hz) that is not capable of 4K at all.

The 1070 Ti has HDMI 2.0b capability, supporting at least 4K60, and even 4K120 + VRR with certain settings.

No, disable HDR on the TV.

Set TV to PC/Console Game Mode.
Might have to switch to a different HDMI port on TV

Tried everything nothing worked, still only staring at 2560 x 1440 as top level under "PC" in the Nvidia Change Resolution screen.

4 HDMI cables (2 are 4K for sure)
no HDR mode on TV nor on Windows
TV is in PC/Console game mode (both on the Home Hub as well as as a picture mode setting)
Tried TV ports 1 and 2; only have 1 port on back of 1070 Ti

At this point, I'll just keep playing at either 1440p/120hz (60fps) or 4K/60hz (30fps) and then decide if I should upgrade my card and or whole PC.

I have the budget for doing just about anything, but I'm pretty frugal so just want to make sure if I decide to spend it's well invested.

I'm confused now. Do you have access to 4K60Hz? If yes, then you are on the right track. You may need to lower the color settings from 10bpc to 8 and 4:4:4 (RGB) to 4:2:0. These changes allowed me to set it to 4K120Hz + VRR on an HDMI 2.0 as I had enough bandwidth.

I do, but it's under the "TV" area settings in the Nvidia control panel, which only offers 60hz so I need to go under the PC section, which shows max 1440p/120hz.

for example, if I choose "Customize" to create a new resolution, I can choose 3840x2160p and for 120hz but then I lose connection to the TV until I hard reset the PC.

You won't lose connection when you do that. If it's some how "out of range" after you apply it. Wait 15 secs and it will revert back.
