is there a setting or tick box to make it take effect at reboot?
are you using MSI afterburner or anything else that affects GPU settings?
Is this the newer Ryzen GPU?

Have you contacted your os/mobo/gpu/chipset manufacturer yet for support on the specific issue? The vast majority of them have M-F toll-free support numbers and paid, (somewhat) friendly, helpful support staff that will assist you with most common issues if you take the time to patiently explain the problem you are having and wait for them to respond.

Sifting through endless forums of topics and simply experimenting with copy-pasted solutions or at-best partial solutions (most-likely by untrained random folks) will inevitably lead to more needless frustration and aggravation. Unless someone is actually willing to post their professional credentials and provide evidence of their experience (you can view my profile & links for that) I, personally, would not trust my expensive, not-easily-replaced equipment with their advice or solutions but what do I know? It also helps if you're willing to provide specifics regarding your system setup but most folks perusing this particular forum are unwilling to trust random internet strangers for that either. Thus back to my first solution at the beginning of this response.

**Btw if your settings are reverting back to their defaults on reboot this could quite simply be an issue with your CMOS battery being dead and the CMOS chip not saving any changes to the mobo settings whatsoever. Or it could also be your needing to flash your mobo's bios so you can update it to the most recent version as well. So it may not have anything to do with AMD at all. Again, you would need to go to the mobo manufacturer's website to verify this information.

I use afterburner purely for monitoring

How does my mobo relate to this? It's an ancient AMD driver issue. My previous card (gtx 1060) saved everything just fine. There are 10500 topics about this bug on different forums

You need to turn off the quick launch, because previously Radeon Wattman, or now integrated in the software Adrenalin, sometimes crashes. That's why it's better to use MSI afterburner for GPU tuning.

As you use Afterburner make sure the Startup button is not selected, if its selected for default settings on Afterburner it'll revert the AMD settings every reboot.

You can just uninstall Afterburner to see if its the problem or not and reinstall later.

:steamfacepalm: You know what: just find yourself a local, certified IT repair technician as I'm certain in these tough economic times they can certainly use the business.

Bingo.I totally forgot that I enabled it long ago. Will check how it behaves with it disabled later
So it turned out to be Aftreburner apply on startup option.Disabled this and finally Adrenalin is saving everything.I'm just a dumbass:steamfacepalm:
