While it paired up like normal and Steam immediately recognized it...I ran into some of the same problems I did using the PS5 controller: heavy lag following my inputs, until it stopped reading them entirely and the controller would shut off.
This tells me the issue might be something deeper. Could there be some driver or setting that's messing up the connection to my controllers? Does DS4Windows install anything that might be doing this? I recall trying to download an Xbox controller driver not too long ago, is there a way to check if that's maybe an issue?

I tried some of the steps there. I don't think I have HidGuardian on my system, but is there a way I can find out for sure?
Almost got my DS4 working again. It does show up under my Windows controllers, but it messes up when on Steam (buttons will lag horribly).
It's got to be something that's messing with the controller connections.
Edit: And when the controller shuts off, the Steam inputs change to Xbox button inputs. Dunno if that helps narrow down the issue.
Do you have any other controllers connected? Because when I turn off the controller in Big Picture mode it switches to keyboard and mouse inputs.
I did once have a problem with HidGuardian too, not sure how it happened but in that case my controller was fully blocked and it was not some halfway, high latency or missing button's bug.
It required a full uninstall and re install of the ds4 windows and drivers.

In none of these screens was I able to get a response to the control inputs, even though it's saying that the controller is connected.
Tick hide DS4 controller so that you dont get two devices on game controllers of which one will not work.

So what I did was go to Device Manager and turn off all Bluetooth devices, including hidden devices. Made sure to un-pair my controllers as well. I also took the 4.0 dongle out of my USB Hub and connected it to a free USB port on the back of my desktop.
Paired my PS5 controller again, and for a while it was working as intended. Steam picked it up and I launched a game to test it out (Sonic CD, which is actually coincidental considering my avatar).
It was working, but it wasn't perfect. I could feel a slight delay in lag input but it was pretty stable. Then after about 5-6 minutes, the lag got really bad, where it would read inputs about 5 seconds or so later. On the positive side, my controller didn't shut off this time.
At this point, there's almost assuredly something that's messing with my Bluetooth connection and/or maybe impacting my system. I don't see anything in Task Manager that's eating away at my CPU or memory, and I doubt I have any viruses or malware, though I'll run Windows Defender to be safe.
Are there any other tests I could run, programs to watch out for, etc?
USB dongles have weak signals and are easily affected.

I have a BT dongle specific for my BT headphones and it's plugged into the same USB hub I use for the BT dongle for my DS4. If they are plugged in next to each other it causes interference. Latency constantly spiking and even on the rare occasion disconnects. Separating each BT dongle by 1 USB port fixes the issue. Do disconnects and no spikes. DS4 latency stays below 10ms
