Some games are unplayable with my PHILIPS TV monitor for this reason...
I'm using a PHILIPS HDTV screen as my PC monitor, and when I open certain video games, like Destiny 2 or the Resident Evil 2 remake, my screen keeps switching between being black and the philips logo showing up. Turning off the TV and then turning it on again doesn't help, and when I try to open the task manager, it goes back to being stuck in this loop when I select it after pressing CTRL+ALT+DEL. The only way to get out of it at that point is to reboot the computer via the power button. Oddly enough, I could play Destiny 2 without this problem until a certain update to the game I can't remember.

Any idea what's causing this and how to fix it?

Try launching those games in lower resolution as see if screen goes black.
What about Windowed or Borderless Windowed mode?

How do you check if that's the case?

take it to a tv repair shop

or test it youself on a console or other device
Could be a display driver crashing issue, depending on how the TV responds when it loses signal.

The thing to try first would be using DDU in safe mode then installing the latest graphic driver.

The model number is 32pht4319/12. It's roughly 4 years old.

Just tried out your suggestion. Unfortunately it didn't have any effect.

does it do the same with a console or other hdmi source?

its a smart tv, may need bga reflow/reball or cap replacemnets

if it works fine til it warms up then does not work til shut off for a while, odds are its bga solder balls are cracked
Most likely an issue with the cable you are using to connect to the display, or an issue with the TV itself.

If you have a different HDMI or whatever you are using you could give that a try.
If you have Nvidia maybe try this. Open your Nvidia Control Panel.
Then set your resolution to: 1080p 1920 x 1080 (it's a setting for TVs etc.)

There are no problems with the TV when using a console. And the TV has never warmed up, shut down and then being unable to turn on for a while.

I've tried using the other HDMI and setting the resolution to 1080p 1920 x 1080 via the nvidia control panel. But I've still had no luck with these games.

Have you ever thought of checking what your GPU is doing when your screen goes black?

Have you tried it with a new HDMI cable that supports the latest standards?

Seems to me it's not your TV but a pc config issue.
