I have exactly the same issue.. and also did all what you did, reinstalled everything including windows, Nvidia, using DDU, and my GPU still stuck at 139mhz.. Were you able to fix the issue?
My temporary fix is to use MSI Kombustor and run the benchmark, the GPU for some reason sometimes goes up to max clockspeed which is 2,000mhz then I use Evga Precision X1 to turn on the "Lock Boost" to keep the GPU running at max speed until I stop gaming.

He got a different GPU since then..
Which he said BEFORE your post...
Hey guys,
I have brougt an broken GPU (GTX 1070 strix OC) with the exact same problem. I dont understand russian but I think I know what the person in the video did. He swapped the power controller for the GPU powerphases out. My GPU uses the uP9511P controller , you can find stuff like this cheap on aliexpress. Luckyly I have such an contoller in new codition lying aound.
After reading all the comments it seems that the controller is slowly going to die and needed to be replaced.
So I will try this out and keep you updated.
Clocks not raising is also a potential driver issue, or something on your PC not allowing the clocks to raise properly.
If you havent tried the DDU method thats the first place to start.
