but the tolerance is +/- 5-10%
There's what the 12v readings are if measured professionally on a 750w version of your PSU. Your readings are a bit higher and not measured professionally, so I wouldn't worry unless you're having hardware problems.
Those voltages are way too high, and when using a good quality PSU, I've never see them that high.
11.9xx - 12.1xx is about the normal ranges
Basically 10% up or down from whatever the actual target is; if its out of that margin, chances are the PSU is junk.
10% up or down for a 12V circuit would be 13.2V to 10.8V. I think the voltages are just fine. 12.288V is 2.4% above.

Yes overall the OP's voltages seem ok, and if was truly an issue, the user/machine would see all sorts of issues if the voltages were in any way "out of whack" so to speak for sure. Best bet is test your system with a few tests that will actually put a proper 100% load on both CPU & GPU while doing some things also with regards to RAM and Disk Usages; if the PC holds up for beyond 30-60 mins test intervals with such tests, then the PC's overall stability should be deemed "OK"
