If you are getting that high of an fps, maybe increase some settings to help improve visuals. Also it's fairly un-realistic to have the ped/vehicle density below approx 50, but lower only if in need of an fps boost.
For GTA4 it's best to not even use the in-game settings, aside from loading into a new game or saved game, go to the visual settings, set to defaults, apply. Then set the correct screen resolution, save. Then exit the game. If the game allows you to run the game with uncapped frames, I don't think you need to worry.
If you want to run the game anyway with 60 fps on a 144 hz monitor, get Rivatuner[www.guru3d.com].
On the left panel choose "Add". Browse to the gta4 executable and choose it. To limit the fps ingame put the number of frames you want to run the game in the "framrate limit" field.
Capping the framerate increases input latency.
There is a benefit having a higher fps than refresh rate, you get the most updated frame etc etc, Linus explains it better in this video https://www.youtube.com/watch?v=OX31kZbAXsA
Pause at 20:15 and you can see shroud goes from averaging 0.6 hits to averaging 6 hits between 60 and 300 fps. I recommend watching the full video
You can of course set the framerate limit in the global profile, it just affects every application.
So it's easier to mantain and set up profiles for specific games, instead of changing or deactivating the global profile each time you want to play the game. Set it up once and you're fine.
Try to play at 30 or 60 frames and what effect it has.
Initial release is even uncapped on consoles
