Turn off the PlayStation 4. Locate the small reset button on the back of the DualShock 4 near the L2 shoulder button. Use a small, unfolded paper-clip or something similar to push the button (the button is inside a tiny hole). Hold the button down for a couple of seconds and release.

Once un-paired, try the Reset process (hold the reset on Controller for about 10-15 secs) then connect it to a PS4 that is powered off already, connecting it via USB Cable, as a means of getting the Console to then pick it up again. Try turning on the Console via the Controller. With the controller connected via USB cable after this reset process, the PS4 should pair it automatically, even though it's on wired connection.

So I guess to summarize:
Use either a different controller or your usb keyboard (press F3 to access the options page) to delete the controller that won't work. After that press the share and PS button at the same time and choose your controller.

Hey I’ve had this issue recently, and I have a wired keyboard and mouse that I usually use for my pc and I hooked them up for the first time ever by the USB ports on the front of the ps4 but now I can’t get passed the screen that says “the PS4 was not turned off properly blah blah blah” which button on the keyboard is mapped to the accept button or “X” ? I never controlled a ps4 with a keyboard before
I'm in a similar situation; I accidentally used both of my DS4 controllers for games on my PC, and despite unpairing and resetting them, they refuse to connect to my PS4 again through wired connection. I managed to find an old USB keyboard and it works with the PS4 just fine, but I have no idea what key(s) to press in place of the PS button in order to get past the opening "Welcome Back to PlayStation" screen. I read somewhere that the Pause/Break key is supposed to take that role, but a quick press does nothing, and a long press just takes me to the power options menu (rest mode, restart, shut down). Any help from those who did have luck getting into their PS4 with a keyboard would be much appreciated.

You mentioned a button on the "front" of the controller. Is there more than one reset button? I tried the only one I know of on the underside while the controller was turned on, like you said. The light flashed white, attempting to connect to something and immediately turned off when I pressed the reset button.
The USB keyboard has been the only thing that works so far, so again, if anyone knows what key(s) to press in place of the PS button, I would much appreciate your help.

if that still fails to pair using another controller or k/m in the ps4 menu
turn off the ps4
hold ps4 power until the 2nd beep
plug in the controller using a micro usb data cable
press ps on the controller
pick reboot
and it should work again
It turns out the USB cable I was using was only capable of charging, not data exchange. I found a better cable and now it works as intended.
