Below you will find solution.
To be honest I don't use it before but I find your question quite interesting.
I'm sorry for writing how to set and restore custom covers (the stuff you know already) but maybe someone will find this useful. BTW It's good explanation how it works step by step and why backup is so easy;)
1. To set custom cover:
- right click on game in library
- choose option: manage->set custom artwork
- choose cover image (for example: game_cover.png)
INFORMATION: I assumed default Steam instalation directory for further explanation.
- chosen cover file (game_cover.png in the example) will be copied to folder: 'C:\Program Files (x86)\Steam\userdata\NUMBER\config\grid\'
- name of the (copied) file in the folder will be GAME_IDp.png
- you can use jpg images as well (copied image will be GAME_IDp.jpg in that case)
2. To restore original one:
- right click on game in library
- choose option: manage->clear custom artwork
WARNING: Created (copied) image will be deleted from 'C:\Program Files (x86)\Steam\userdata\NUMBER\config\grid\' folder.
3. Backup covers for future.
If you backup above folder with all content and re-install Steam etc. just restore it. I think all covers should be customs again. All covers will be customs again.
INFORMATION: I don't test it but it looks like there is no information about paths in configuration files (like for example for screenshots). The only information I found is about grid itself. That's why I think (but I'm not sure for 100%) it should work.

The thing is... My suggestion is... I do not want to backup & restore the covers manually. I want them to be stored on the (Valve) server (Steam Cloud, maybe), not on my HDD/SSD only.
Not really. If you put the folder in which the covers are saved back where it was before after a new installation or a change of OS, it will automatically take over all the cover adjustments you have made. You don't have to make them all individually again.
