Or you can turn off community generated content, the store, profiles etc. in it's entirety if so desired. It's not perfect, but it's there for use and does prevent a lot of these issue's.
1) Adult Sexual Only Content is OFF by default.
2) Responsible parents use Family View to restrict what their children can see.
3) Irresponsible parents leave access to their PC where children can see real life porn.
It is odd that you play GTA games which shows children banks being robbed, people being shot etc but you see no issue with those games.
Parents are responsible for ensuring their kids browse as "safe" as the parents want it to be. All kinds of settings via Family View can be used by parents.
The "porn" games are disabled by default, content is blurred. To me that's enough.
It's because of the cultural zeitgeist. Nowadays extreme hyperviolence is good, porn is bad.
Man, I remember when Mortal Kombat and DOOM led to the creation of the ♥♥♥♥♥♥♥ ESRB because extreme hyperviolence in games was considered bad. Now kids less than 12 are playing Call of Duty and GTA... slinging slurs and ♥♥♥♥.
