[Valve index] Plastic cable holder on HMD
So the little plastic tab that holds the cable on the left side of the HMD just above the speaker on my HMD broke.

I have quite a large head meaning the clamp is almost fully extended when I wear my HMD. So this cable would see tension. That and I also play things that require violent movements (Beatsaber for instance) which shakes this part around a lot while it's in tension. So that's probably what broke it. This seems to be an issue of design which could be addressed immediately with newer batches of HMDs or in future iterations of the index. Maybe increasing the thickness of the weakest part where it broke.

As a simple fix, you could wrap the cord in black electrical tape and onto the HMD side. Are you getting a new cord from Valve? What's the cost?

Although my head is normal sized :) ...I did two things to prevent this...

[1] I unclipped the plastic guide on the VERY back that holds the wire. (being mindful to note which way is up) I then opened the HMD head band to fit the largest head it could. Then I used black electrical tape to secure the wire to the plastic clip at the perfect length. Then clipped it back in place. If done right this looks good and works great.

[2] I took the head strap out of the front, then I wrap a thick rubber band around the cord then feed the head strap through the rubber band, then back to the front. Now the rubber band takes part of the stress instead of only the plastic parts. This doesn't look great, but helps distribute the load better too.

I did this to assure I don't tug the wire out of the HMD plug, but it would prevent your situation too.
