It does. It happens to me to many games over many years and everytime it causes a lot of questions in the game specific forums. For example the free DLC's for Warhammer Total War are starting the game when you install them and the today released free DLC for City Skylines does it as well.
No it's just starting the game (and installs the dlc as well) without any warning, checkbox or other uncommon screen.
It only happens when you have the main game installed btw. If you don't have the main game installed and download a free dlc for specific game it's just added to your library without starting the game.
It always causes a lot of confusion in games forums since people think the DLC isn't downloaded/installed since the game start running when clicking the download/install dlc button.

Allowing downloads during gameplay is checked indeed.
But still I don't like a game start running when I click a "download" button.
That's why I suggest this in the forums. If I download a dlc for Warhammer (to play later) I don't want Steam runs Warhammer without asking me.
I created the main post at march 22 in 2017 so I do not expect a fix anytime soon myself.
At least nice someone found the threat and bumped it after years that's why I got notified, but yeah, the problem still remains.
Whoever thought it was a good idea to combine the two should be shot....
I really hope the staffs do something about their most important transaction features.
