There is no separate Combined Operations game.
Not familiar with the pack or CO. Appreciate it.
1. Download/install "ARMA 2" via Steam, start the game and play bootcamp for a minute or two then quit game.
2. Download/install "ARMA 2 Operation Arrowhead" via Steam, start the game and play bootcamp for a minute or two then quit game.
3. Exit Steam.
4. Navigate to the Arma 2 Operation Arrowhead folder on your computer eg.
C:\Program Files (x86)\Steam\SteamApps\common\Arma 2 Operation Arrowhead
5. Find the file called "_runA2CO" (Windows Command Script).
6. Right click "_runA2CO" and send to desktop as a shortcut.
7. Go to your desktop and right click the new "_runA2CO" shortcut you just made and select EDIT from the right click menu.
8. Look towards the end of the document for the "runs" line - this is the line we will edit (it appears just before ENDLOCAL).
9. Mine looks like this:
(my mods are CBA, I44 and COWarmod with -nosplash at the end):

10. Once you have modified the runs line to your requirements, click SAVE and use the shortcut to launch Arma 2 as COMBINED OPERATIONS!!!
PS. Your mods can be placed in to the same folder as the Arma 2 Operation Arrowhead exe
In my case I have copied the following modfolders in to the same folder as ARMA2OA.exe
@I44
@CBA_CO
@COWarModI44
Hope this helps somebody - took me a while to figure it out!
