Title kinda says it all, is there any way to overlay the skype cam chat window over a fullscreen game.

Perhaps if Skype had an "always on top" feature it would work, but you would have to move it out of the way/minimize it yourself.
I think XFire actually integrates with Steam in the way you're thinking.
