For one, patches often do change existing files, so while the size of the download itself is small, the size of affected files is not, even though you will hardly see a change in hard drive use.
Secondly, Steam only shows the size of the last uploaded patch in the "preview". If you missed a big one in the meantime, it may be much more than you expected.
(I know, I had a third thing in mind, when I started typing, but I can't remember what it was...)
A game requires all the files to be read in a smooth flow and wont if sections are simply cut off and a new piece added. .
