most of the times, it will be based on community reports on appropriate profile picture.
if you got enough reports, there will be a penalty ( i'm not sure what is tho).
which is why each people have different toleration on which appropriate and which are not.

for myself, I dont really pay attention to profile pic, as well as ascii written name lol

sometimes profile pic are hard to see since it is small anyway.

skins and some "seducing" pic should be fine,
but pure porn or genitals obviously not.

however, there are alot more disturbing pic that far worse than just a simple porn.
such are insulting religion, racism, black politics and such.

some people with simple minded (and bad taste) like me, who likes "family guy", won't mind any of those.
however, normal people would be disturbed/insulted
