or as in - you notice no difference in gameplay.
when you change settings, does your game re-load from desktop (depending on setting)?
you can always try to manually edit your cfg file - changing the render options and wut not.
maybe your graphics options are staying on reload of game? Is your steamfolder / or game files set to read only? I use read only alot- to stop games from changing certain things back on me, like mw2 would try to do with ping bars.
I have found the biggest improvement came when I realized that the problem related to the ping I have(about 30ms) and the lobbies I was playing in - some are up to 175ms. When the lag compensation system built in to MW3 clicks in, those with low ping get hammered.
I picked up the following tip in a number of blogs but I cant remember the blogger's handle, so apologies for the failure to attribute the source.
You need to look at the "maxping" parameter in the Steam library on your machine. When you do so, you will find that the programmers figure that the game will play with lobbies up to 800ms and man, that is death. - in the game - it's literal!
I have found a significant improvement by lowering the "maxping" to 50 ms. it is not cheating as all I am doing is reducing the number of lobbies I can play in. I understand that the console players have the option of selecting the quality of the lobby (in terms of ping), you don't on a PC - not that I could find, anyway. It does mean that I have to wait quite frequently to get a game - but it's worth it.
So have a go, change maxping and see how you go. it hasn't made me into a super player but I am now more competitive head to head and I can usually understand why I lost out in a competition - wrong weapon parameters etc. - much less frustrating.
From memory, you need to step through the following on your C drive - or wherever Steam is resident - Steam/Steam apps/player2/ mw3 config (am a bit hazy on the last bit). then look through the code and find max ping - change it using notepad and you are done.
Good luck.

How to Optimize Matchmaking in Call of Duty: Ghosts to Decrease Online Lagg
Hello, 9 years later this game is unbearable to play online in any coop setting with a friend. Both of us lag really bad and its borderline ass. Do you know what specific setting you have to change under the config file to fix this?
