You can click the little person-shaped icon in the top right to see which profile items he has equipped. In this case it looks like he's using the Dying Light 2 profile which makes the boxes more transparent.

If you mean showcases, you get one showcase slot for every 10 levels. Accounts under level 10 can't add a showcase, so you'll need to level up.

Penny Hiya and Happy New year my first time on the community page but I can help :steamhappy:you if you go to store you'll see Points Shop go in there and use your Steam points from there you can get all the FANCY stuff ;)
