Only way I know that could happen is if the number was reported stolen to the card company... I seriously do not know how else that could happen.
Which is why I’m so confused, the card was a gift for her birthday, I remember her opening it and I’d assume if the card was stolen it wouldn’t even be activated rendering the funds useless

I have one more scenario which would end like this....
The payment method used to purchase the gift card was disputed. Same outcome.
Having said that a chargeback has nothing to do with 'abusing refund mechanics' as you put it. A message about refund abuse would only be given if your girlfriend had requested multiple refunds in a short space of time (nobody outside Valve knows the exact trigger thresholds).
A chargeback is an entirely different thing. Post the full message.
