The Witcher 3 specifically crashes quite often. Occasionally the game will get laggy, or it will make a long buzzing sound and then the monitor goes to sleep. Lately an error message will show up saying "Input Signal Out of Range, change resolution to 1920x1080- 60Hz. I do have these settings in place already, however.
It doesn't take longer than 15 minutes for the Witcher 3 to turn to a black screen and shut down.
I've also had issues with my monitor going to sleep randomly while playing any game, and after it turns on the game will not open again.
Here's my current specs (only difference is I have 8 gigs of RAM and I'm running 64-bit)

I'd also go with the fresh install option and uncheck the Geforce (un)Experience(d).
352.86 and 353.06 are full of TDR crashes for me.
Mostly because 353.06 was more of a hotfix for the GTX 980 Ti.
Some people have gotten the crashes less frequent by disabling the Hardware Acceleration & unchecking Nvidia HD Audio upon clean installation. Also uncheck 3rd party On-Screen Displays such as MSI Afterburner's, if you have them on.
