Does that seem like the mapping you are experiencing?
If so, back in Steam Settings > Controller, disable 'Enable Guide Button Chords for controllers', and see if things are different.

Don't worry I think you'd get it, but really you could skip checking the Guide Button Chord layout and just toggle-off 'Enable Guide Button Chords for controllers' and see if the issue persists or not.

I remapped the whole controller, everything is mapped correctly, yet every button now is the escape key for some reason!
Does this mean you are remapping the Guide Button layout? If that is correct, I don't want you to be doing that, but just view the layout to see if it looked like the source of the unwanted actions and then disable it elsewhere.
If you're not remapping the Guide Button layout, where are you remapping things?
I'm not seeing escape anywhere, but I set it to R3, should I disable that?
Steam > Settings > Controller > Guide Button Chord Layout > gear icon > Reset Controller Layout to Default
And then disable it:
Steam > Settings > Controller > disable 'Enable Guide Button Chords for controllers'
So with this layout out of the way, see how things are this time.
For info: The Guide Button Chord layout is active while the PS button is held (providing things are working correctly with Steam/the controller).
If things are still weird, we can check out the Desktop Layout.
This layout is active when you are outside of a game that Steam knows is running, so if you're experiencing the odd mappings while playing Steam games I wouldn't suspect this layout but we'll disable it anyway.
Steam > Settings > Controller > Desktop Layout > Edit > Disable Steam Input
Are things still weird?
