Best graphics card for (both heavily modded) skyrim and minecraft, FPS like titanfall and COD ghosts?

I currently have an AMD Radeon HD 7560D 4GB graphics card but I'm finding that it's not always handeling some games (as mentioned in the title) as well as I'd like.

Any suggestions for a better graphics card?

The 7560D is not a graphics card, this GPU is built into the CPU so technically you don't have a graphics card at all.

"Best" is obviously most expensive. You need to specify a budget and describe your current system specs if you want any meaningful advice.

I personally prefer Nvidia chips. So maybe something like GTX760 or 770.
AMD has similar: Radeon R9 270x and 280x.
CoD Ghost doesn't run well on pc anyways.

I currently have:
8gb ddr3 ram
ASRockASSM-DGS motherboard
AMD A8 quad core 1600k 15.6ghz proccessor

My budget is about £150-£200.

You need to first check you have a spare 6pin PCIE power connector from your power supply, otherwise you would need to buy a new one of those as well. It would also need to provide 500w+ of power.
