Valve should add that information to the store.
It's in the Steam Support database (I have updated my post above to include that info), but as far as I know it's not present on the purchase page itself.
That depends on the particular card in question. There should be a URL on the back of the card. Try looking there.
On the back of the card is a website. You login there using the CC # and expiration date. After that, you register your name and address with the website. Then all doors will open for you.
