It only happens when im playing games...
feel free to scan and optimized all driver and bios update.
reduce steam setting, to test if its your pc dont like them. ask own game forum
test with other hardware even. im sure your friend might help you test with it.
try see it as ruleout and try proof its steam only issue,
do note this is a user forum , we are not support here and we are not steam white defender.
Gather proof to write to steam support , and claim fix your sound issue in game, takes time.

we are not hear to prevent you from contacting steam support, but with no data or proof i bet they will say we cant help or we dont make sound drivers.
but if they do screw access to it , then you will be right, but im sure in my point they dont make any sound card driver and they never will, they use what system tell them.
i mean im sure you have seen sounds settings in Friends & Chat ( output device )
then steam suppport will look investigate this even further , most tech person will say whats going on here. and send it to tech department , most will not say user own mistake then it work 3 other place with echo test in skype and Discord selftest and windows own sound check.
if this is what you mean then you might be on to something.
and problem is also many use maybe discord or skype or other ventrilo app and not steam own so they dont know.
and do have newest sound card driver install. not the first time a dxdiag tell a tech person newest driver not install and that is why.

because you just told us even a reinstall OS did not help. and thats not good.

feel ftree to update all driver and bios, and do test with skype echo test and discord selftest and windows system test, maybe steam do have own ingame issue , 2 user complain atm over same thingy, dont mean its same but why not.
reduce steam settings to see if steam takes to much recource away from your hardware.
send steam support a ticket , could also be a good idea.
I did send steam a video and ticket of the issue. I also went on forums that are being flooded with people having this issue. Reddit, github, guru3d, tomshardware just to name a few. People are saying we just have to wait for the next steam update to fix it, but as far as i know that may never happen. As of right now i do have a temporary fix that i have been telling others, if you type in "sound" and have the "recording" windows open in the background, you will not have this issue.
if you notice there is only Friends & chat with sound settings rest is from system, so steam most have done something wrong sense more and more get ingame sound issue.
its like the point at wrong layer or output device, and even that should be impossible.
but i bet if you mention all of this.
try safe boot mode also , so you dont have interference issue from other app, even that is fare fetch out, but who knows. but again most will say work all other places then it seem to be a steam ingame steam sound issue.
and maybe thats why steam devs have hard to understand it ,they dont make driver and they dont have any thing other then use windows system , what is there.
ps.
you could also say steam have lost its Resources down the road with all the change and ingame sound over network is the cause , they cant deliver it, even that is possible.
broken app, to proof this is hard , depend on each person network in top of this.
do not have wifi or vpn and have nearest dl sever if this is not routed to nearset steam server or follow DL connection again only steam have the card on the hands here.
dont forgt that sound is data and goes though what to where automatic nearest or user DL server across the other part of globe, just because i have a idea dont make it so only steam knows here.

steam ports ( im sure steam sounds use one of them ) just in case you dont use NAT Router or block it through windows firewall, most never had any issue with this but just incase some do.
