Port fowarding

HELP!! Port forwarding
Hi i havent done this before and am looking for help with and explanation.
I have my router open and the virtual server i know that once the values are put in i press apply
bout this is the part i dont understand and i assume im doing it wrong because im unable to get to the main menu once i have done so it comes up with error 42

I found these
Uplay PC:
TCP: 80, 443, 14000, 14008, 14020, 14021, 14022, 14023 and 14024.

i have no idea for example if i need to put them as 80-80, 80-14021 or 80-14024
or if i even have to put all those number in to as many slots as i can fit them as in double up 80-80 443-443 and so on.

Also i have
Game Ports:
UDP: 3074 and 6015

now for this my question is pretty much the same is it 3074-3074 and 6015-6015
or is it 3074-6015?

now without these changes i can get to the main menu look at operators and such and even que but nothing happens after that i just cant get into any game or join a friends squad.

Depends on your hardware how it looks like, but if to visualize somehow:

If it asks for that LAN IP, you can find it via cmd.exe -> ipconfig/all
