Alright this might help: http://windows7themes.net/en-us/fix-atikmpag-sys-atikmdag-sys-blue-screen-errors-bsod/
Ive just refollowed these steps, I'll give it a couple days and report back on how its doing:P

Did you do those re-installs clean?
Driver un-install clean up, good fourms over there too.

Could have been an unstable over-clock the whole time though, it may notice issues in some games but not in others. Some games just don't play well with over-clocked cards.
And yes improperly clocking your card can kill it, and lessen its lifespan, so it would be no faults of the card.

yes I did, and i tried it without the oc and the same results happened
The problem continues :( bluescreened during fallout:NV

Maybe a voltage issue with card, MB BIOS? Did you just over-clock just from AMD CCC software or did you change anything in your BIOS too?

Are your CPU and RAM standard clock?

Power Supply issues.?

What other background software/ tasks are running?

I know that things like https://justgetflux.com/ can cause some issues BSOD and gaming.

Short of testing the card in another PC to see it will do the same, idk.
It's not the overclock, its been bluescreening sincw before the oc, Power supply seems fine, I think the best solution would be to save for a new GPU :(
