Steam controller shipment/versand information

Hello, im sorry if its at the false place.
So. i want to buy a steam controller and let it send to me but i dont understand the shipment-informations form. i know every field without "delivery address", "delivery address, 2nd row" and "telephone number".
I know that i have to fill in in one of the fields my adress such as my house and street number but for what is the second?
And what i have to fill in at the telephone number field? My landline number or mobile phone?

Pls help me. i want to get the winter sale offer for it :D
