1. A combo router/modem is going to have a lot of issues versus two individual devices and if you are able to use your own devices for this will be a lot better then the companies device that you have to rent.
2. You will need to check the settings in the device to make sure everything is set up correctly and if you can you will need to call them to determine the issue as this could be a issue with the device or on their end.
3. Just because you are paying for a set speed does not guarantee you will get that speed and even more so during peak usage hours if they have oversold in that area without having the needed bandwidth.
