Report a translation problem
Are you using DS4Windows or other input mapper?
If using DS4W or other mapper, quit them.
If connected over Bluetooth, disable PlayStation Configuration Support (or quit Steam) and power cycle the controller.
Then see if the controls are OK at gamepad-tester.com
It is bluetooth, and when I disable Playstation configuration support, everything stops working on the controller.
If you had, see how it goes with Steam not running. It could be Steam is still switching the controller to it's other mode even with PS Config Support disabled (it's doing that for DualSense controllers but not read of people facing it for the DS4).
Quit Steam > power off/on controller > see if the tester is reading things
If no inputs are read then we check the way Tev brought up.
