1. Disable bakground crap
2. Update driver
3. Lower graphics settings
If everything else fails, buy a better graphcs card.

Nope! :D those are the useless tips people give to someone, this one is so simple that you dont have to be a genious to do it. :D, and plus i've never seen anyone talk about till now.

Those are the real tips. Everything else is either a placebo or treating symptonts instead of causes.

Here it is: Click the right mouse button while on desktop and go to >Display Settings. Now depending on your lag, you lower the screen resolution of your screen up to 800X600 if necessary. For example, let's say you're playing Dark Souls and you've already put DSfix but you're going around 10-15 fps, changing the resolution to 800X600 can get you to 25-30 FPS, i know this cause the same thing happened to me on Dark Souls :D

Tried it. Didn't work. The Witcher 3 doesn't care. And why should it? The desktop isn't even drawn when a fullscreen app is on!

1) you should put the same resolution in the game as your screen (forgot to mention it)
2) even if you're in a game, the the pixels you use for are still the same as your screen, Example: You cant play a 4K game in a 1366X720 screen, well you can, but it wont be 4K. So if you put the resolution to 800X600, the game must adapt to your screen, since it cant change it. I dont know the witcher 3, but in all games i tried there's always been a great difference :D

1. You also forgot that I mentioned that
2. Speaking about placebo, Voodoo or similar nonsense. You're not making any sense. In fact, you're downright lying! Of course can the game change the screen resolution!

Dude think for a second, lets say your screen is 1366X720 (laptop), then you start a game or video that goes more than that, you're still gonna see the same resolution as your screen. And a Game does change the resolution, not your monitors', but the resolution of the window you're in (even if fullscreen). Now lets get to the point, the less pixels there are, the less stress on the game/pc, so if you lower your resolution, the game is bound to scale its self, if you have played dark souls, you can notice that if you lower the monitor resolution below 800X600, the game crashes, since it doesnt support lower than that, so games DONT DOWNSCALE O UPSCALE YOUR RESOLUTION, they adapt to it. ReBoot, i know you're fixated with this argument and that you're gonna keep pointing out everything, can you atleast confirm your theories before doing that? i dont wanna write another poem :D
