Have you right mouse clicked Call of Duty: Modern Warfare 3 - Multiplayer and gone to Properties -> Local Files -> Verify Integrity of Game Cache?
1. Open cmd.exe [You can hold down the Windows key and press R and type that in and enter]
2. In cmd.exe, type in ipconfig/all and hit enter
3. Pick out your DHCP and IPv4 -addresses
4. Open your web-browser, type in the DHCP -address (typically 192.168.x.x)
5. See your router manufacturer & model on the page (incase you didn't know them before), Google for directions to the port forward menu if not too clear on the router's settings as this varies greatly with different hardware.
6. Typically in NAT > Port Forward start applying new rules
Apply, with some older hardware you'll still have to reboot the router (turn it off and turn it back on for the changes to take place, but most likely not.)
Make sure on your Windows firewall iw5mp.exe is in the exceptions, its location is at C:\Program Files (x86)\Steam\SteamApps\common\Call of Duty: Modern Warfare 3\
minus the (x86) part from the Program Files -folder if you're on a 32-bit operating system.
