''Duplicate Product Code''

Hi all I try to install the game Call of duty black ops 1 I see an error message which says '' Duplicate Product Code '' Then I can not continue the installation ....
I forgot my password and user name of the first installation .... it was 5 years ago
And I forgot where I bought the game .... I can not find any help anywhere in the internet. Help me!

Then you are out of luck and need to buy the game again if you want it on the new account.
Once a key has been activated its forever bound to that specific account.

someone tell me that I need to prove to steam that I have my real game box but how I will do thats?

Open a ticket on the support page about your lost account.
https://help.steampowered.com/en/
Include a photo of your game box with the visible cd key and include it.
Tell them everything you remember about the lost account.
If you are lucky its enough so they will give you the login credentials to retrieve it.

No one told you to go to the Activision website. They can't help you to recover a lost Steam account.

but if I want my game I need to enter to activision website and to proved them about my code and if I will have a lucky they will give me another code to install my game

You need to recover your Steam account, not the game itself.
