In order to find In-voice after it disappearing for some damn reason, you can find it on Steams' Big Picture mode.
Settings(top right cog), then go to Voice, under the Audio tab.
Here's what I found about In my particular issue:
Steam has the 'Voice Recording Device' set to a PLAYBACK?!
Which means, they're using my output device, rather than an input.
my mic is set as the "Default Device" & "Default Communication Device" and all other playback/recording devices have been disabled. I have tried disabling audio enhancements, changing my Steam password, reinstalling Apex & Steam, turning the Microphone Access off then back on, updating my audio drivers, letting Steam record my mic, followed every tip listed on Steam Support page regarding microphone issues, uninstalled GeForce Experience as well.
I'm stuck here.

Audio output is playback tab, this is where you find speakers, headphones, tv/montior, etc....
Audio input is recording tab, this is where you find microphone, software recording, stream mix, and etc...
For output select your output you want to use that you want to hear audio from, if using speakers, select it, then click set default button that it, you have a green check mark on it.
For input, select your microphone want to use, then click set default button, that it, you have a green check mark on it.
Relaunch your game & apps so they can take affect, as some apps and games don't auto detect changes.
Have you check in-game settings to sure that it using mic, as well ensure that audio settings not mess with such as incoming, and out going voice chat?
Because you said you made new account, and worked, this tells me you did something to in-game settings, or possible you may be muted & deaf on game server if it a custom server.
I'm aware on what the Input and output is for. If you look at the screenshot, Steam has the output set for the Voice Recording Device.
Which means, it's not even picking up my input. It's using my output Instead.
If I clicked on 'Detect Audio Devices', It hits me with the prompt that they haven't added the feature yet.
I use a shure sm7b with a GOXLR.
It's an xlr. I've already tried unplugging and plugging back in .
Ahh I kinda suspected you used that, I'm not sure what to do then, sorry.
