PS4 controller constantly disconnecting (hear USB sound over and over when it does this)
It doesn't disconnect if I'm still or just leave it plugged in not moving. I think it's the loose micro USB port on the controller/bad design. Does anyone else have this issue? What can I do about it?

Sorry i don`t have the answer. Just want to say that i have same problem. I can play fine for some time but it can randomly disconnect. Can hear sound like when you disconnect device manually. I can`t use it for few seconds and then message appears in Steam overlay that controller is connected now or something like that.
Wired original DS4 controller that came with PS4. I tried different USB ports and nothing worked. I played without this problem for a month or two but now this is happening again. In fgs Ranked matches it`s unacceptable. Also i can`t remove it safely because there is no icon on taskbar to remove device. I can see it on "Bluetooth and other devices" page as Wireless Controller, under audio devices. I`m using PS4 option in Steam BP mode.

I bought an Xbox One controller with wireless adapter and a play and charge kit and it’s solved all my problems.
