2) Sometimes the connection is severed and you will need to set up sharing all over again.
I've played Fallout 4 from the friend's library for a couple of hours before, now none of the shared games work which includes Fallout 4. The Steam shared games that I didn't install are completely absent from my library list.
Could you expand on point 2?

Yup. Just happens sometimes that the Family Share connection bugs out and has to be set up again.
Next time, after you set it up, go into your Steam installation folder and there into the "config" folder and make a backup copy of the config.vdf .
That file has all the saved Family Share connections and some other configurations.
If the connection ever bugs again just exit Steam , copy and overwrite with that config.vdf backup and after restarting Steam it should work again.
