Avg. Game Completion Rate
Does having a 0% achievement score on a game affect your Avg. Game Completion Rate or does a game only affect the score after at least one achievement is earned?

This. The percentage is not counted until one achievement is earned.

You can have a 100% on one game, and another game that you've played but not gotten an achievement and you will still have 100% rate.
