You want it set to Adaptive or Fast.
maybe Adaptive( half refresh rate) depending on the game if you're fluctuating fps doesn't exceed monitor refresh rate.

3) Really? fiddling with registry really shouldn't be needed. But then windows sometimes does weird stuff so I suppose it's possible.....
Ok fair enough if you use vsync in other games, yes leave it enabled in Nvidia control panel. Majority of people who play FPS games and aspecially competitively won't leave it on.
Yes registry was needed for me like I said above after the latest update.

At least add the suggestion of creating a " System Restore " point before attempting your method.
Right Click on "START" and set its value from the default "3" to "4".
