For example, CCleaner - if you don't exclude Steam with it.
I have other accounts without SGMA tied to them, and as long as nothing removes my cookies and I tick this box upon login,
And I don't have this box ticked under [Steam > Settings > Account]
It doesn't ask for the Steam Guard code upon login.

Steam is a browser too. All games are executed with a browser protocol of steam://rungameid/%app-ID% and if you use the client console, you can practically open any website with Steam.
If you have CCleaner or a similar sort, make sure to exclude Steam with it.
That on the other hand demands the authenticator / backup code from me if I use [Change User...] on the desktop client.
"Is this still your current email address? User@domain Yes No, update
Your email address is used to confirm purchases and help you manage access to your Steam account"
Then I don't know, unfortunately. :/ Haven't seen any clear patterns as to when these messages usually pop up, as I've had it come up at different times on my other accounts than the current one.
Could imagine it has to do with some levels of activity, but -- that's about as good as my guess can go, and one would imagine anyone's guess is that good.
