Or like a real life photograph of some gorey thing?

Depending on how it's used i'd guess that something from a (non modded) game would likely not result in a ban. But that's just my guess. It's possible if it's too explicit (even if from a game), like if it's a game that comes with a MA/18+/Age Verify rating... then maybe you shouldn't put something like that on your profile.

Steam is for users age 13+. Any material that would not be suited for users that age is not allowed to be displayed on profiles, artworks and so on. If you want to display things like that you need to set your profile to private or just for friends.
